The following document will list the type of personal information we collect and how we store, handle and process such information. 

Me and My Therapy CIC are the data controller and are responsible for the safe keeping and disposal of such data after an agreed time period. 

Paypal and Stripe are the data processors who collect payments as a third-party payment processor and are subject to their own privacy policies. 

You as a consumer are the data subjects.

With your consent, you will store and record your child or young person’s name and date of birth for the purpose of personalising the box, in this case your child will also be referred to as the data subject.

We currently collect and process the following information:

· Personal identifiers, contacts for the purpose of shipping and communication, and characteristics (for example, name and contact details) 

· Payment 

By agreeing to our terms and conditions and privacy notice at the checkout when purchasing a product and/or subscribing to a subscription plan, you give your informed consent for your data to be stored and used as detailed within this notice.

How we get the personal information and why we have it

Most of the personal information we process is provided to us directly by you for one of the following reasons:

· You are a parent/carer and want to subscribe to a six month ‘Me and My’ box for children and young people

· You are a young person, aged 13 or over and want to subscribe to a six month ‘Me and My’ box for yourself

· You are a professional and want to subscribe to a six month ‘Me and My’ box for children and young people, either for you to support them with or to pass to them to complete with their parents/carers.

· You are a parent/carer and want to purchase a one or more well-being boxes

· You are a young person, aged 13 or over and want to purchase a one or more well-being boxes

· You are a professional and want to purchase a one or more well-being boxes, either directly to you or on their behalf

· You want to subscribe to our mailing list to receive relevant updates specific to the topic of bereavement or well-being

· You make contact regarding further support for a child or young person and may require some additional information

We also receive personal information indirectly, from the following sources in the following scenarios:

· Where you choose to follow our social media, including commenting on posts published by Me and My Therapy CIC.

· Information gained from social media will not be saved or held by Me and My Therapy Ltd.

We will not share your information with any other organisation, agency or third-party.

Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are: 

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting hello@meandmytherapy.co.uk

(b) We have a contractual obligation.

(c) We have a legal obligation.

(d) We have a vital interest.

(e) We need it to perform a public task.

(f) We have a legitimate interest.

How we store your personal information 

Your information is securely stored on our private and confidential data system, only accessed by Me and My Therapy Directors (x2). 

We keep names and addresses for a six-month period and dispose of the details at the point of the final postage labels are completed. If a one-time purchase, we will dispose of these details immediately after postage labels are completed.  Data will be deleted via secure means using a shredding confidential waste system. 

With obtained permission, we will keep email addresses for mailing purposes. 

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information. 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
